What is the FIB-4 index?
A non-invasive estimate of hepatic fibrosis using age, AST, ALT, and platelet count. It helps decide who needs further fibrosis assessment.
How is FIB-4 interpreted?
< 1.45 makes advanced fibrosis (F3–F4) unlikely; > 3.25 makes it likely; 1.45–3.25 is indeterminate and warrants elastography or biopsy.
In which diseases is FIB-4 used?
Originally HIV/HCV co-infection; now widely used in NAFLD/MASLD, chronic hepatitis B/C, and alcohol-related liver disease.
What are FIB-4 limitations?
Lower specificity in patients > 65 years (age-adjusted cut-offs are recommended) and unreliable with acute hepatitis or thrombocytopenia from other causes.
