How to use
- Select each of the five ABCD² components for your patient.
- The total score (0–7) and 2-day stroke risk update instantly.
- Use alongside urgent TIA work-up — do not delay imaging based on a low score.
Frequently asked questions
What is the ABCD² score?
A 7-point score that estimates short-term stroke risk after a transient ischemic attack (TIA), using Age, Blood pressure, Clinical features, Duration, and Diabetes.
What ABCD² score needs admission?
Many guidelines admit or urgently work up patients with a score ≥ 4 (or any patient with crescendo TIA, AF, or carotid stenosis regardless of score).
What is the 2-day stroke risk by band?
Score 0–3 ≈ 1.0%, 4–5 ≈ 4.1%, 6–7 ≈ 8.1% risk of stroke within 2 days of the TIA.
Is ABCD² still recommended?
It is a useful triage adjunct but should not replace urgent specialist assessment and imaging; some guidelines now favour rapid TIA-clinic pathways for all patients.
