How to use

  1. Assess RASS first; if arousable, evaluate the four CAM-ICU features.
  2. Tick each feature that is present.
  3. CAM-ICU is positive when Feature 1 and 2 plus Feature 3 or 4 are present.

What is the CAM-ICU?

The Confusion Assessment Method for the ICU is a validated bedside tool to detect delirium in critically ill patients, including those who are mechanically ventilated.

When is the CAM-ICU positive?

When Feature 1 (acute change or fluctuating mental status) AND Feature 2 (inattention) are present, together with either Feature 3 (altered level of consciousness, RASS ≠ 0) OR Feature 4 (disorganized thinking).

Should I assess sedation first?

Yes — assess RASS first. If the patient is deeply sedated or unarousable (RASS −4/−5), the CAM-ICU cannot be performed; reassess when arousable.

Does a negative CAM-ICU exclude delirium?

A single negative assessment does not rule out delirium over time — perform it at least once per shift because delirium fluctuates.
