What is the R-ISS?
The Revised International Staging System for multiple myeloma combines the ISS stage (albumin, β2-microglobulin), high-risk FISH cytogenetics, and serum LDH into three prognostic stages.
How are the R-ISS stages defined?
Stage I = ISS I + standard-risk cytogenetics + normal LDH; Stage III = ISS III + (high-risk cytogenetics or high LDH); Stage II = all other combinations.
What is high-risk cytogenetics?
Presence of del(17p), t(4;14), or t(14;16) by interphase FISH.
What is the ISS stage based on?
ISS I: β2-microglobulin < 3.5 mg/L and albumin ≥ 3.5 g/dL; ISS III: β2-microglobulin ≥ 5.5 mg/L; ISS II: neither I nor III.
