How to use

  1. Select the PPS level that best matches the patient across ambulation, activity, self-care, intake, and consciousness.
  2. The score (0–100) and survival category update instantly.
  3. Re-assess regularly — declining PPS over days is an important prognostic signal.

Frequently asked questions

What is the PPS scale?

The Palliative Performance Scale (PPSv2) is an 11-level (0–100, step 10) functional assessment tool used in hospice and palliative care to guide prognosis and resource allocation.

How does PPS predict survival?

In hospice populations, PPS ≤ 30 has been associated with median survival of days to weeks; PPS 40–60 weeks to months; PPS ≥ 70 months or longer. Always use alongside clinical judgment.

PPS vs Karnofsky?

PPS extends the Karnofsky scale with additional dimensions (ambulation, intake, consciousness) that better capture decline in palliative populations.

When is PPS assessed?

At hospice admission, after major changes in condition, and at regular intervals (often weekly or with each visit).
